My test request form is missing or there is a problem with it

All PCR and antibody tests come with a request form, this should indicate the type of test you are due to take.  If the test request form seems like it is for a different test or is missing please contact us straight away so we can provide you with a replacement.

If you have sent in a PCR test or antibody test without a request form, please contact us immediately and let us know - without a request form the lab will not be able to associate your sample with our account so we won't automatically get your results and there may be a delay.
